ABSTRACT

OBJECTIVE: We conducted a study of the effect of an organic solvent on the failure of ovarian function after exposure to 2-bromopropane for 7 years. METHODS: We conducted a study on 25 female workers in a manufactory who were exposed to 2-bromopropane in 1994. Some of them experienced premature ovarian failure. We have investigated their recoveries from ovarian function and checked LH, FSH, E2, BMD for 7 years of period. RESULTS: 16 among 25 workers experienced amenorrhea, but the rest of them did not report amenorrhea. In 10 out of the 16 amenorrhea patients, recovery from amenorrhea were seen, but 6 did not recover from amenorrhea. Through ovarian biopsy, it was observed in the amenorrhea patients that mature follicles were lost and only primordial follicles were present. Through HRT, gradual decrease in FSH and increase in E2. in the amenorrhea patients were found. Also, their BMD were decreased, but gradually increased with female hormone replacement therapy. CONCLUSION: The study confirms that the exposure to 2-bromopropane leads to the serious ovarian toxicity and ovarian failure as well. In such case, the failure of ovarian function, which is reversible change, can be recovered after long-term periods. A significant factor which affect ovarian failure and recovery from ovarian function is patient's age. In industrial environment, physical and psychological damage due to the use of and exposure to chemical materials will likely increase. Hence, more studies of industrial materials used in working conditions are needed.

ABSTRACT

PURPOSE: There has been various reports on the inverse association between coffee consumption and serum gamma-glutamyltransferase(GGT) activity in the general population of Western countries. However, this association is uncertain in the communities in which coffee consumption is less popular. So we tried to identify this association in men of rural area in Korea. METHODS: We examined the association between coffee consumption and serum GGT activity in cross-sectional study involving 379 male population health examinees aged 40 years and older from rural areas in Kyungnam prefecture in Korea. Those who perceived their health status as poor were excluded. Possible confounding effects of age, body mass index, alcohol consumption and cigarette smoking were controlled by multiple regression analysis and analysis of covariance. RESULTS: In multiple regression analysis, serum GGT activity was positively associated with alcohol intake, body mass index(P<0.001) and was negatively associated with coffee consumption(P=0.022), whereas no significant association was observed in cigarette smoking(P=0.094). Adjusted serum GGT activity level at the coffee consumption level of 0, one and two or more cups a day was 37.6 U/l, 35.9 U/l and 26.8 U/l respectively(Ptrend=0.008). CONCLUSION: Decreased serum GGT activity appears to be associated with coffee consumption in men of rural area, and further studies on the interrelationships among coffee consumption, alcohol intake and obesity will be necessary.

ABSTRACT

OBJECTIVES: Copper and Zinc, the trace elements of a living body, take a part in immunologic mechanism and induce the pulmonary fibrosis which is the pathologic progress of pneumoconiosis. This study was performed to assist the identification of the pathologic process of pulmonary fibrosis in pneumoconiosis. METHODS: The subjects of this study was 200 diagnosed persons as pneumoconiosis, of whom 100 persons was the visitors in the outpatients departments of pneumoconiosis and 100 admissive pneumoconiosis patients. The serum copper and zinc are measured and com pared. RESULTS: The mean serum copper concentration of visitor group was significantly higher than that of control group, and that of stage V3 subgroup of visitor group was highest because of their advanced fibrosis. But, there was not the difference of serum copper concentration between patient and control group. We thought that the reason was poor nutritional and health status of the patient. The mean of serum zinc concentration of visitor and patient control was lower than that of control group. As the degree of pulmonary fibrosis was more severe from stage Vl, V2, V3 of visitor to stage Pl, P2, P3 of patient, the mean of serum zinc concentration was lower. The ratio of serum copper/zinc of visitor and patient was higher than that of control, and that of patient was higher than that of visitor. Those results showed that the ratio of serum copper/zinc ratio more greatly expressed the degree of progression of pneumoconiosis than only copper or zinc concentration by adjustment of nutritional and health status of the patient. CONCLUSIONS: As above study results, the degree of pulmonary fibrosis of pneumoconiosis could be estimated by means of serum copper/zinc ratio.

ABSTRACT

This study was carried out to evaluate the effect of noise exposure and age on the changes of group mean hearing threshold level(HTL) over 7-year period. For this purpose, consecutive annual audiometric tests were performed from 1991 to 1997. among 718 male employees of a metal product manufacturing factory. The subjects were divided four groups as follows according to noise level category(NLC). NLC-I officer, exposed noise level was under 60 dB(A) of time weighted average(TWA) NLC-JI technical assistant or engineer, they exposed to workplace noise occasionally NLC-IJJ worker, exposed noise level was below 85 dB(A) of TWA. wore hearing protection device(earmuff or earplug) NLC-IV worker, exposed noise level was over 85 dB(A) of TWA, wore hearing protection device(earmuff and earplug). The results were as follows 1. The improvement of group mean HTL was continued until the fifth year, showing the peak at the third year. The magnitudes of this learning effect were 1.5 - 4.6 dB. 2. The mean HTL of each age group tended to increase after the third year and the tendency was more prominent at 4000 Hz. 3. In noise exposed group(NLC-II, III, and IV), mean HTL increased from the third year after decreasing, meanwhile, in noise free group(NLC-I). it was few changed for the entire period. Among the noise exposure group, the mean HTL of NLC-IV was lower than that of NLC-III and NLC-IV. 4. After learning effect, the mean increase of HTL in noise free group(NLC-I) was 0.4-1.7 dB that suggests aging effect, and that in noise exposure group(NLC-ll, III, and IV) was 0.9 -4.1 dB that suggests noise effect. 5. Statistical analysis of the general linear model implicated that the effect of age was statistically significant at 500, 3000, 4000 and 6000Hz. and the effect of noise exposure was statistically significant at all frequencies except 6000Hz. However, the age * noise interaction was not significant at all frequencies. From these results, it was concluded that the effect of age and noise exposure seems to affect the mean HTL independently and these two factors contribute to an additive effect for the mean HTL change. Furthermore, more concerns should be needed for hearing conservation of low level exposures without any specific protection.

ABSTRACT

In order to evaluate the cytogenetic hazard among hospital workers potentially exposed to low dose of radiation, the analysis of chromosome aberrations(CA) and sister chromatid exchanges(SCE) in lymphocytes were performed in 79 hospital workers and 79 non-exposed workers. The mean frequency of chromosomal exchange and deletion(respectively, 0.20X10-2/cell and 0.39X10(-2)/cell) in the exposed group were significantly higher than those(0.07X10(-2)/cell and 0.23X10-2/cell) in control group. The frequency of sister chromatid exchanges was 5.04/cell in the control vs. 6.57/cell in the exposed group. There were also significant differences in the mean frequencies of CA and SCE adjusted for age, sex, smoking, drinking between two groups. There were no evidence of significant increase of CA and SCE according to the department or duration of employment. But the frequency of cells having chromosome aberration was significantly higher in the exposed group than in the control group related to duration of employment. There was no dose-effect relationship between the cumulative doses and the frequency of CA and SCE. But in the case of last 1 yr cumulative dose, there were evidence of significant dose-dependant increase of chromosome type CA and percentage of cells with aberration. The result suggest that there is cytogenetic hazard in risk group like hospital workers handling low dose radiation. And the analysis CA and SCE are useful biological indicators for the exposure of low dose level of radiation.

ABSTRACT

The authors examined human immunity indicators such as CD4, CD8, Natural Killer (NK) cell, IgG, IgA, and IgM, and heavy metals such as Pb, Cr, Cd to evaluate the health hazard of welders. We selected total 126 male subjects, 60 welders who had more than 5 year working history as an exposure group, and 66 office workers and medical college students as a control group. This immunity evaluation approach is the first newly designed study that has never been tried in past studios on welder's heath evaluation. And the results were as follows. 1. Total lymphocyte count of peripheral blood was significantly higher in welder group, 2,615+/-650 ea/microliter compared with 2,368+/-681 ea/microliter in control group (p or =0.05). 3. IgG, IgA, and IgM in welder group were 15.9 mg/dl, 2.7 mg/dl, and 1.5 mg/dl, respectively, and 15.6 mg/dl, 2.9 mg/dl and 1.7 mg/dl in control group. There was no statistical significance between two groups(p> or =0.05). 4. Blood lead, cadmium and chromium concentrations were 4.2+/-1.8 microgram/dl, 0.4+/-0.38 microgram/dl, 1.2+/-0.6 microgram/dl in welder group and 3.2+/-1.7 microgram/dl, 0.44+/-0.29 microgram/dl, 1.1+/-0.6 microgram/dl in control group, respectively, and these value were within normal ranges.

ABSTRACT

This is a cross-sectional study to evaluate the serum lipid levels of children living in Pusan in 1996. The distribution of age is from 2 years old to 12 years old. In female children mean total cholesterol is 171.4+/-26.2mg/dl, triglyceride is 104.7+/-50.6mg/dl, HDL-cholesterol is 54.4+/-14.8mg/dl, and LDL-cholesterol is 95.4+/-32.9mg/dl. In male children mean total cholesterol is 167.9+/-25.2mg/dl, triglyceride is 90.6+/-45.5mg/dl, HDL-cholesterol is 55.4+/-11.7mg/dll, and LDL-cholesterol is 94.4+/-23.6mg/dl. The percentile of serum lipid levels is measured in children. The 95th percentile of serum total cholesterol is 210mg/dl in male children, and 214mg/dl in female children. And, the 95th percentile of serum triglyceride is 184mg/dl in male children, and 191mg/dl in female children. And, the 95th percentile of LDL-cholesterol is 133mg/dl in male children, and 135mg/dl in female children. Serum total cholesterol is positively related to age(r=0.18), height(r=0.08), weight(r=0.17), obesity index(r=0.12), and negatively related to father's education level(r=-0.13), mother's education level(r=-0.13). Serum triglyceride is positively related to weight(r=0.23), age(r=0.31) and negatively related to father's education level(r=-0.12), mother's education level(r=-0.18). Serum HDL-C was positively related to mother's education level(r=0.07) and negatively related to height(r=-0.12), weight(r=-0.09). Conclusively, the serum lipid levels of children living in Pusan is generally so high that the family and school must try to control the serum lipid levels.

ABSTRACT

The author surveyed overall obesity indicies and factors concerned with obesity such as dietary intake, physical activity, stress and life style with the subject of doctors. The number of subjects was total 508 with 396 men and 112 women. They were subgrouped into surgical part, medical part and service and basic part by speciality. And also subgrouped into intern and resident, pay doctor, and practitioner by working type. The results were as follows. 1) Obesity indices: BMI of total doctor was 23.1+/-2.8, and WHR was 0.87+/-0.08 and overweight prevalence(BMI>25.0) was 23.6 %. It was within normal limit but slightly over the Korean standard. The degree of obesity indices of subgroups by speciality was "surgical part > medical part > service and basic part", and by working type was "practitioner > pay doctor > intern and resident". 2) Dietary intake and Physical activity: Average dietary intake was 2148+/-451 kcal/day. The degree of dietary intake by speciality was "surgical part > medical part > service and basic part". By working type it was "practitioner > pay doctor > intern and resident". Average physical activity was 29+/-5 METs/day. The degree of physical activity also showed similar pattern. But there was no significant difference among each groups. 3) Comparison between over-weight and non-over-weight group: The items that showed significant difference between two groups were dietary intake, skip breakfast, regular exercise, smoking, heavy drinking, chronic disease etc.

ABSTRACT

Helicobacter pylori is now recognized as causative agent of chronic gastritis and peptic ulcer, and strongly associated with development of gastric carcinoma. With development of sensitive and specific serologic tests to identify individuals infected with Helicobacter pylori, the epidemiologic study of this diseases has been investigated. But it's transmission route is not established, yet. The purpose of this study is to measure the prevalence of Helicobacter pylori infection in healthy children and young adults and to evaluate related factors for Helicobacter pylori infection in Korea. The seroprevalence of Ig G antibodies to Helicobacter pylori was determined using a Enzyme Linked Immunosorbent Assay and we obtained the information, such as demographic characteristics, monthly household income, numbers of family members in the house, numbers of persons using same room, type of house, and type of drinking water through the questionnaire survey. The observed overall seropositivity rate was 25.7%. The rate is increased progressively from 5.8% in the age group 1~3 years to 44.4% in the age group 20~29 years(chi-square for trend, p<0.001). Especially, the rate increased steeply from 6.5% in the age group 4~6 years to 20.8% in the age group 7~9 years, and this suggested that elementary school age was the major acquisition time of Helicobacter pylori infection. In multivariate logistic regression model, age, numbers of family members in the house, and type of house was statistically significant variables for Helicobacter pylori infection. Each odds ratio(95% CI) were as follows; base to age group 1~9 years, age group 10~19 years 3.6(2.0~6.4), age group 20~29 years 7.3(4.1~13.1) and base to group of 1~3 family members, groups of 4~5 family members 2.1(1.1~4.0), group of 6 or more family members 2.7(1.3~5.4) and base to apartment, single and multihouse 1.9(1.1~3.5). Sex, monthly household income, numbers of persons using same room, and type of drinking water was not statistically significant for Helicobacter pylori infection.

ABSTRACT

To investigate the factors which affecting the cognitive impairment of the 60 or more age group, the authors surveyed for the subjects in some area of Kyungnam Province. 201 studied subjects were tested for cognitive function with mini-mental state examination(MMSE). Information on demographic characteristics and life style has been collected through direct interview. The concentration of Al and Ca of subject's drinking water, which might be related with cognition, was measured by Inductively Coupled Argon Plasma Spectrometer. The main results were summarized as follows. 1. The prevalence rate of cognitive impairment was 18.4% in male and 45.2% in female and this sexual difference was statistically significant(p=0.03). And the uneducated or illiterated showed significantly high prevalence rate of cognitive impairment(p=0.02). 2. In stratified analysis by sex and education year, we can not see significant trend indicating the neurotoxic effects of aluminum and protective effects of calcium to the cognitive function(p>0.05). 3. The correlation between the concentration of aluminum in drinking water and the MMSE score in whole subjects showed weak negative relationship(r=-0.066). But there was no statistical significance(p=0.434).

ABSTRACT

This study was carried out to analyse the food intake and associated factors of the urban poor elderly by comparing poor district, Unbong rental apartment in Bonsong 2 dong with other areas in Pusan. 135 elderlies(men 36, women 99) in Unbong rental apartment 136 elderlies(men 45, women 91 ) in the other areas were investigated during the period of March to August in l994. The assumption that the study area represented poor district was satisfied because the age and sex distribution was not significantly different, and the income of the study area was significantly lower than that of the control area. The variables of hospitalized in previous 12 month, gastrointestinal problem, alcohol drinking, cigarette smoking did not differ significantly. But the variables of chronic disease, take medicine, perceive, vitamin supplement differed significantly between two groups. Therefore some factors associated with health state in the study area are worse than those of the control areas. At most of all variables, nutrients intake of the study area did not reach the recommended dietary allowances(RDA) for Koreans, and that nutrient intakes of the study area were significantly lower than those of the control area. The hypothesis of this study that nutrient status depends on economical status was proved. As for the score of nutritional knowledge, the study area was significantly lower than the control area. But as for the score of nutritional behavior, two areas were not significantly different. The latter is counter result of our hypothesis, owing to the effect of the confounding factors including education etc. As for the correlation of variables, not only economic status and educational level, but the score of nutritional knowledge effects strongly on nutrient status in the study area, the poor district. Therefore, adequate nutritional education to the elderly in the poor district should be considered.

ABSTRACT

This survey was performed to obtain the basic information for the prevention and management of occupational low back injury(LBI). The subjects of this survey were 952 workers(male, 892;female, 60) who had taken occupational LBI in some industries of Pusan area from January 1 to December 31, 1991. Observation period was 2.6 years from the beginning to the end of medical treatment. The obtained results were summarized as follows; 1. The proportion of LBI workers was 15.9% of the workers who had taken any occupational injuries and 0.32% of all workers in this surveyed area. 2. 8.0% of the workers had taken LBI on the 1st day of employment and 55.2% of the workers were within one year, 91.4% of the workers were within 10 years according to cumulative frequency distribution. 3. Handling of heavy objects was the most common cause of LBI(32.0%) and fall down(26.9%) and slip down(16.3%) were the next in order. 4. The most common causes of both lumbar sprain and HNP were handling of heavy objects and that of lumbar fracture was fall down. 5. The mean duration for medical treatment of LBI workers was 143 days. 6. The total direct compensation cost for LBI workers was 6,736 million Won and the proportion of medical, resting, disability compensation costs were 25.0%, 37.0%, 38.0% respectively. 7. The percentages of retreated workers and disabled workers were 11.9%(113 persons) and 22.9%(218 persons) of total LBI workers respectively.

ABSTRACT

In the field of industrial health, it is important to establish the efficient system for health management of workers. The management of health information of workers using data base will contribute to such a system. In this context, the utility of method using Mahalanobis distance(D(2)), the general probability distance in multivariate analysis, for decision of the results of health examination was evaluated. Six items of test among the health examination data of 278 prospective employee was used to calculate D(2). Sensitively and specificity of decision method using D(2) with 99% normal range was 95.8% and 90.2% respectively as compared with decision by doctor. This finding suggests the utility of method using D2 for decision of the results of mass screening for industrial workers.

ABSTRACT

To provide the basic for assessment of renal dysfunction related to silicosis, urinary N-acetyl-beta-D-glucosaminidase(NAG) activity known as a sensitive markers for early renal damage were measured in 58 silicosis patients, and control subjects of 40 pulmonary tuberculosis patients and 51 official workers. The results were summarized as follows. 1. The values of blood urea nitrogen and serum creatinine in all subjects were within reference limits. But the mean value of urinary NAG activity(7.25+/-7.31 U/g creatinine) was beyond reference value and more sensitive test than others. 2. The mean value of urinary NAG activity in silicosis group was 11.98+/-9.05 U/g creatinine and significantly higher than in tuberculosis and healthy group(p0.05). 3. The value of NAG activity in tuberculosis had a tendency to be increased according to severity of disease, but that was not significant(p>0.05). The value of NAG activity was increased significantly by use of nephrotoxic antituberculosis drugs(p0.05). 5. After excluding the users of nephrotoxic antituberculosis drugs, the mean values of NAG activity in healthy control and in tuberculosis control were same as 3.63 U/g creatinine and 3.60 U/g creatinine, respectively. But the mean value of NAG activity in silicosis group was remarkably increased as 10.90 U/g creatinine(p<0.01). As above results, even though there are no abnormal findings in screening renal function test, silicosis can be related with renal dysfunction. And it will be very useful to apply urinary NAG activity in health management of workers exposed to dust.

ABSTRACT

In order to study the relationship between life-style and serum lipids level in young adults, the author measured the concentration of serum total cholesterol (TC), triglyceride (TG) and high density lipoprotein cholesterol (HDL-C) and investigated age, relative weight, weekly alcohol intake and daily cigarette smoking through questionnaire in 310 male workers aged 20 to 39, in Ulsan area. The results obtained were as follows: 1. In the factors being able to influence serum lipids level there was negative correlation between age and alcohol intake, and the positive corrrelation between age and relative weight, alcohol intake and relative weight, alcohol intake and smoking with the strongest correlation between alcohol intake and smoking. 2. In univariate analysis, mean total cholesterol concentration were significantly different according to age, smoking and relative weight; mean triglyceride concentration were significantly different according to relative weight only; mean HDL-C concentration were significantly different according to alcohol intake alone. 3. In non-drinkers, HDL-C concentration of smokers were significantly lower than that of nonsmokers but triglyceride concentration of smokers were significant higher. And in drinkers, total cholesterol concentration of smokers was significantly higher than that of non-smokers. 4. In multiple regression analysis, significant independent variables were relative weight, age and smoking in the total cholesterol concentration, and relative weight, age and alcohol intake in the triglyceride concentration, and alcohol intake, relative weight and smoking in the HDL-C concentration. By these independent variables, total variation in each dependent variable was explained 7.9%, 17.6% and 7.4% respectively.

ABSTRACT

The author investigated the values of total serum cholesterol and its correlation with the physical factors to have potential as a line in the chain of basic investigation to establish maternal health program. The study group was composed of 167 healthy non-pregnant and 209 pregnant women in the age of 20-39 residing in Pusan area. The obtained results were as follows: 1. No significant differences were found in height and systolic and diastolic blood pressure in both groups except for the elevation of Broca's index by gaining the weight in pregnant women. 2. There were significant differences in the total serum cholesterol level of the healthy and pregnant women as 165.9 and 212.6 mg/dl, respectively, not showing the differences in the age. Total serum cholesterol values in both group followed approximately normal distrubution curve. 3. The significant correlation to the values of total serum cholesterol were found between weight and Broca's index in healthy women and between weight, Broca's index and duration of pregnancy in pregnant women. 4. The values of total serum cholesterol in pregnant women showed increasing tendency with the duration of pregnancy as 168.1 in 12 weeks and below, 209.6 mg/dl in 13-26 weeks and 235.4 mg/dl in 27 weeks and above group. Total serum cholesterol values by duration of pregnancy followed normal distribution curve.

ABSTRACT

In order to exmaine the mutagenicity of cadmium dichloride the author studied the induction of sister chromatid exchanges in chinese hamster ovary K1 cells which treated with cadmium dichloride at various concentrations. The results obtained were as follows: 1. In cells treated with 10(-4) M cadmium dichloride, a small number of cells were visable but no mitosis was bound. 2. The frequencies of sister chromatid exchanges in cells treated with 10(-5) M and 10(-6) M cadmium dichloride as 10.7 +/- 1.9 and 8.3 +/- 2.1, respectively, were significantly increased for control (6.0 +/- 2.3). (p<0.05). 3. There were dose-dependent relationship between the concentration of cadmium dichloride and frequency of sister chromatid exchanges in cells treated with cadmium dichloride at concentration ranging from 10(-5) to 10(-7) M.

ABSTRACT

In order to know the possibility of utilizing the sister chromatid exchanges as an index which could evaliiate the effect of organic solvents on Lhe health in industrial workers, the authors Studied the effects of the inductivity of sister chromatid exchanges in peripheral lymphocyces from 90 female workers espoxed to organic solvents .and 20 lion-exposed female workers. The results obtained were as follows: 1. The frequency of sister chromatid exchanges in peripheral iympnocytes from 90 female workers exposed to organic solvents was significantly increased in comparison with 20 coatiol subiect. 2. The frequency of sister chromatid exchanges was significantly Increased in the workers who were employed in the manufacture of plastic materials than the other manufactures. 3. There were no significant differences in the frequency of sister chrornatid exchanges by carriera of the exposed workers. 4. The correlation between the frequency of sister chromatid exchanges and urinary hippuric acid was significant with the coefficient of 0.5902 showing Y=1.867X+15.188 in which Y indicate the frequency of sister chromatid exchanges per cell and X indicate the urinary hippuric acid concentration by g/l.

ABSTRACT

From May lst to November 30th, 1971, a study was conducted by authors on the health status of industrial workers through applying the modified Cornell Medical Index forwards a total of 4,565 labourers working in Busan City. The obtained results were as follows : 1. The largest number of industries and labourers was in Busanjin Gu, 9 and 2,386 (52.2%), respectively. 2. Total numbers of labourers subjected to the survey were 2,355 (51.6% in male, 2,210(48.4%) in female. 3. By age and sex distribution, the largest was seen with the age group of 25-34, 1,260 in male, otherwise with under the age group of 24, 1,994 in female labourers. 4. The highest average number of complaints by section among all age groups and industries was as follows ; In case of male labourers, textile was the highest with numbering 8.7 (35-39 age group) in the section of physical complaints, in the section of mental and total complaints, food industry was the highest with numbering 8.2 and 16.4, respectively, both under 19 age group, otherwise in case of female labourers, food industry was the highest with numbering 12.3 (physical), 10.3(mental) and 22.7 (total complaints) with all in 20-24 age group. 5. The lowest average number of complaints by section among all age groups and industries was as follows; In case of male labourers, other industry was the lowest with numbering 0.4 (55and over) in the physical, 0.3 (19and under) in the mental and 0.7 (55and over) in the section of total complaints, otherwise incase of female labourers, basic metal was the lowest in the all sections with all in 50-54 age groups.
